Dragon Ball Super Card Game – Store Information Form

If you’re interested in hosting the Dragon Ball Super Card Game in your stores, you can use this form to register your store information. Stores may also use this form to update their judge information on file.

Stores should nominate players who they feel would be trustworthy and passionate judges within their community for certification. Knowledge and love of the Dragon Ball Super card game are a pre-requisite for success.

To be certified as a Judge, players must meet the following requirements:

  • Must be 18 years or older;
  • Have been nominated by a current tournament store;
  • Have a valid Working with Children check;
  • Have passed the online Dragon Ball Super Judge certification test;
  • Have completed TAK Games’ Judge Application Form.

Judges will receive judge packs on a regular basis shipped to their venue as appreciation for their assistance.
